The Human Support Group provides reablement services to you as step-down services following a hospital stay, but they can also be step-up services, aiming to provide the necessary support to prevent any further deterioration in your health that could lead to a hospital stay.
Reablement services are time-limited, often to periods of six to twelve weeks. However, given the importance of personalising support to achieve your own goals and aims, contracting and commissioning these services is most effectively done not on the basis of time periods and tasks, but on the outcomes desired for you to be able to live as independently as possible following an accident, or illness and resulting stay in hospital.
The role of a carer in a Customer’s reablement can be a delicate one.
Your friends and family are not always aware of the reablement ethos, of maximising independence and may want to step in and do things for you. We therefore encourage carers to help them to change their own behaviour to support you to be more independent.
